Correction:
I think you just print out the form, fill it, then pass it to Ms. LEE of Student Council, Block B. I wasn't given any instructions on how to go about... just this only, so the safest would be to pass it to Ms. Lee... the reason why i assigned this task to Ah O, Bala and Celine so that they can assist you should any confusion surface... this workshop is not compulsory to go... so don't worry yea! =)
